1. That I authorize Daufenbach Camera, LLC to charge my Credit or Debit card for the purpose of rental fees, for equipment, sales item purchases, services, damage waiver fees, a 30% rental deposit and other charges by Daufenbach Camera, LLC.
2. That, in the event, any rental equipment be stolen, lost damaged or returned late I also authorize Daufenbach Camera to settle these costs with this card also.
3. That I have received and am satisfied with all of the products or services associated with all of these transactions.
4. That I have no dispute and agree to waive any dispute rights that I might otherwise have in connection with these transactions.
5. That I agree to pay for these charges pursuant to my agreement with the card issuer.
6. That I understand and agree to Daufenbach Camera's Rental Terms & Conditions which are attached to all Rental Agreements as well as published on www.daufenbachcamera.com
7. That I agree to allow Daufenbach Camera to use the above care/account as a condition of extending credit terms on an open account our as a guarantee of insurance deductibles for claims that may arise.

I understand that credit card funding decisions are being made and I agree that the above merchant's credit card processor may rely upon my statement on this letter.

The undersigned certifies that the above information is correct and authorizes the above-named banks and /or trade references to disclose any information on creditworthiness. The individual signing below for the entity (the "Customer") submitting this Credit Application hereby guarantees absolutely and unconditionally the prompt payment when due of all indebtedness and other obligations of every kind and nature of the Customer to DC, no or hereafter existing, and the undersigned individual agrees to pay all expenses (including without limitation attorney's fees and legal costs and expenses) paid or incurred by DC in endeavoring to collect any amounts due from the Customer and in enforcing this guaranty.
